Job Description

As a Case Manager at Kimwell Nursing Home, you will play a vital role in ensuring the well-being and care coordination of our residents. Your responsibilities will include:

  • Assessing the needs of residents and developing personalized care plans
  • Coordinating with healthcare professionals, families, and residents to ensure comprehensive care
  • Providing guidance and support to residents and their families
  • Monitoring and evaluating the effectiveness of care plans
  • Advocating for the needs and rights of residents
  • Collaborating with interdisciplinary teams to ensure effective communication and coordination of care

Requirements

We are seeking candidates who meet the following requirements:

  • A degree in Nursing, Social Work, or a related field
  • Prior experience working as a case manager in a healthcare or geriatric setting
  • Strong clinical and behavioral expertise
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Knowledge of relevant regulations and best practices
