Job Description

As a Case Manager at Kimwell Nursing Home, you will play a crucial role in coordinating care and services for our residents. Your primary responsibilities will include:

  • Conducting comprehensive assessments of each resident's needs, preferences, and goals
  • Developing individualized care plans in collaboration with the interdisciplinary team
  • Coordinating and overseeing the implementation of care plans
  • Advocating for the residents' rights and ensuring their needs are met
  • Collaborating with healthcare professionals, service providers, and family members
  • Ensuring accurate and timely documentation of residents' care and progress
  • Providing counseling and support to residents and their families

Requirements

To be considered for the Case Manager position, you must meet the following requirements:

  1. Bachelor's or Master's degree in Social Work, Nursing, Psychology, or a related field
  2. Valid professional license or certification (e.g., LCSW, RN, LMHC)
  3. Prior experience in case management or a similar role
