Join Our Team as a Case Manager

Are you passionate about improving the lives of seniors and contributing to their overall health and well-being? Kimwell Nursing Home is currently seeking a motivated and experienced Case Manager to join our team in Kansas City, MO, US.

As a Case Manager, you will play a crucial role in coordinating and advocating for the needs of our residents. Your responsibilities will include:

1. Care Planning and Assessment

Working closely with our residents, their families, and healthcare professionals, you will conduct comprehensive assessments to determine their individual needs and develop personalized care plans. By evaluating physical, emotional, and social aspects, you will ensure the highest level of care and support.

2. Coordination and Communication

Collaborating with various healthcare providers, you will coordinate medical appointments, therapies, and necessary interventions. By maintaining open communication channels, you will update families on the progress and make recommendations for any adjustments in the care plan, ensuring optimal outcomes.

3. Advocacy and Support

As a strong advocate for our residents, you will be their voice and ensure their rights and preferences are respected. You'll work closely with the interdisciplinary team to address any concerns, resolve conflicts, and provide ongoing support to both residents and their families.

4. Documentation and Evaluation

In compliance with regulatory standards, you will accurately document all care-related information and maintain detailed records. Regular evaluations of the care plans will be conducted to ensure effectiveness and make necessary adjustments to meet changing needs.

Requirements

To excel in this role, the ideal candidate should meet the following requirements:

  • Possess a bachelor's degree in nursing or a related field.
  • Hold a current state license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Kansas City, MO, US.
  • Demonstrate a minimum of 3 years of experience in case management or a related healthcare field.
  • Exhibit strong communication, organizational, and problem-solving skills.
  • Have a solid understanding of geriatric and aging care principles and practices.
  • Be compassionate, empathetic, and dedicated to providing exceptional care to seniors.
