About the Home Health RN Case Manager Position

As a Home Health RN Case Manager, you will play a crucial role in providing comprehensive nursing care to our homebound patients. Your duties will include:

  • Assessing patient health needs and creating individualized care plans
  • Administering medications and treatments
  • Monitoring patient progress and adjusting care plans as necessary
  • Collaborating with interdisciplinary teams to ensure holistic patient care
  • Providing patient and family education
  • Coordinating community resources and healthcare services

Requirements and Qualifications

To be considered for the Home Health RN Case Manager position, you must meet the following criteria:

  1. Valid registered nursing license in the state of Arizona, US
  2. Bachelor's degree in Nursing (BSN) or related field
  3. Prior experience in home health, geriatric, or aging care
  4. Excellent interpersonal and communication skills
  5. Strong organizational and time management abilities

  7. Ability to work independently and as part of a team
