Job Description
As a Case Manager Foster Care at Kimwell Nursing Home, you will play a vital role in ensuring the well-being and welfare of foster care recipients. Working closely with both the foster care recipients and their families, you will assess their needs, develop care plans, and coordinate services to meet their unique requirements.
Responsibilities
- Conduct comprehensive assessments of foster care recipients' physical, cognitive, and emotional well-being
- Develop customized care plans based on assessment findings and individual needs
- Coordinate and facilitate referrals to appropriate healthcare professionals and community resources
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary

Qualifications
- Bachelor's degree in social work, nursing, or a related field
- Previous experience in case management, preferably in a foster care or healthcare setting
- Strong knowledge of foster care regulations and best practices
- Excellent communication and interpersonal skills
- Ability to assess and prioritize multiple needs and demands
- Detail-oriented and highly organized
