Job Responsibilities

As a Case Manager at Kimwell Nursing Home, you will play a key role in ensuring our residents receive the highest level of care and support. Your primary responsibilities will include:

  • Conducting comprehensive assessments of residents' needs and developing individualized care plans
  • Collaborating with healthcare professionals, families, and residents to create effective care strategies
  • Ensure proper documentation and maintenance of residents' records
  • Coordinate and monitor the implementation of care plans
  • Participate in interdisciplinary meetings to provide input on residents' progress and needs
  • Advocate for residents' rights and well-being

Qualifications

To be considered for the position of Case Manager, candidates must meet the following requirements:

  • Minimum of a Bachelor's degree in Social Work, Nursing, or a related field
  • Valid state license or certification as required
  • Prior experience in case management, preferably in a long-term care or geriatric setting
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Knowledge of relevant regulations and guidelines
