You will work closely with our interdisciplinary team, including physicians, therapists, and social workers, to ensure that our residents receive comprehensive and holistic care.

Responsibilities

As a Case Manager, your responsibilities will include:

  • Conducting thorough assessments of residents' physical, mental, and emotional well-being
  • Developing individualized care plans based on the assessments
  • Coordinating with healthcare professionals to implement and monitor care plans
  • Providing ongoing support and guidance to residents and their families
  • Advocating for residents' needs and rights
  • Collaborating with community resources to ensure a smooth transition for residents
  • Participating in interdisciplinary team meetings and discussions
  • Maintaining accurate and up-to-date documentation

Qualifications

To be successful in this role, you should possess the following qualifications:

  • Minimum of a Bachelor's degree in Nursing or related field
  • Valid state licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Proven experience as a Case Manager in a healthcare setting
  • Strong knowledge of geriatric and aging care
  • Excellent communication and interpersonal skills
  • Ability to multitask and prioritize workload
  • Detail-oriented with strong organizational skills
  • Compassionate and patient-centered approach to care
