Your Role as a Family Support Specialist

As a Family Support Specialist at Kimwell Nursing Home, you will play a pivotal role in providing assistance and guidance to families and individuals navigating the challenges of aging and long-term care. Your role will involve understanding the unique needs of each resident and their family members, and offering emotional support, resources, and personalized care plans to ensure the highest level of comfort and well-being.

Key Responsibilities

  • Provide emotional support and counseling to residents and their families
  • Conduct thorough assessments to identify individual needs and goals
  • Collaborate with interdisciplinary teams to develop personalized care plans
  • Assist families in accessing community resources and support services
  • Advocate for residents' rights and maintain their dignity
  • Educate families on available options and facilitate decision-making processes
  • Coordinate and facilitate support groups and educational programs
  • Ensure documentation and record keeping are accurate and up-to-date

Qualifications and Requirements

To excel in this position, you should possess the following:

  • A minimum of a Bachelor's degree in Social Work or a related field
  • Prior experience in geriatric care or working with older adults and their families
  • Strong interpersonal skills and the ability to empathize with others
  • Excellent communication and problem-solving abilities
  • Knowledge of community resources and support services
  • Proficient in documentation and record keeping
  • Ability to work effectively within a multidisciplinary team
