Role and Responsibilities

As a Case Management Supervisor, you will play a crucial role in ensuring the delivery of top-notch care to our residents. Your main responsibilities will include:

  • Supervising a dedicated team of case managers to ensure high-quality care coordination and management for our residents
  • Developing and implementing individualized care plans, in collaboration with our interdisciplinary team
  • Evaluating and monitoring the effectiveness of care plans, making necessary adjustments to optimize resident outcomes
  • Providing guidance, support, and mentoring to case managers, fostering professional growth and development
  • Collaborating with physicians, therapists, and other medical professionals to ensure seamless coordination of care
  • Assessing resident needs, conducting regular assessments and reassessments to ensure care plans remain current and effective
  • Adhering to all regulatory guidelines, policies, and procedures to maintain compliance

Qualifications

To qualify for the Case Management Supervisor position, the following qualifications are required:

  • Bachelor's degree in Nursing (BSN) or related field, required
  • Previous experience in case management and leadership roles, preferably in a geriatric or aging care setting
  • Strong knowledge of care coordination principles, geriatric care practices, and relevant regulations
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with healthcare professionals, residents, and their families
  • Proven organizational and problem-solving abilities
  • Current and unrestricted registered nursing (RN) license in the state of Georgia
