About the Family Centered Clinician position

As a Family Centered Clinician at Kimwell Nursing Home, you will play a vital role in providing comprehensive care and support to our residents and their families. Your primary responsibility will be to serve as a liaison between the medical team, residents, and their loved ones, ensuring effective communication and coordination of care.

Responsibilities

As a Family Centered Clinician, you will be responsible for:

  • Conducting thorough assessments to identify the physical, emotional, and psychological needs of residents and their families
  • Developing individualized care plans and goals in collaboration with the interdisciplinary team
  • Providing counseling and support to patients and families, addressing their concerns and helping them navigate the healthcare system
  • Coordinating with external healthcare providers and community resources to enhance the overall well-being of residents
  • Educating families on illness management, treatment options, and available support services
  • Advocating for resident rights and ensuring their preferences are respected
  • Facilitating family meetings and support groups to promote open communication and shared decision-making

Qualifications

To succeed in this role, you should possess the following qualifications:

  • A degree in Social Work, Nursing, Psychology, or a related field
  • Valid licensure or certification as required by the state
  • Prior experience in geriatric care or a similar setting
  • Strong interpersonal and communication skills
  • Empathy and compassion towards elderly patients and their families
  • Ability to work collaboratively in a multidisciplinary team
  • Proficiency in electronic medical record systems
  • Strong organizational and time management skills
