About FCHP Mini Grant Program

The FCHP Mini Grant Program is designed to provide financial support to individuals, organizations, and groups working on initiatives related to geriatric care, health, and aging. Eligibility Criteria

To be eligible for the FCHP Mini Grant Program, applicants must meet the following criteria:

  • Applicants must be based in the local community.
  • Projects should align with Kimwell Nursing Home's commitment to promoting geriatric care, health, and aging.
  • Preference will be given to initiatives that demonstrate a positive impact on the well-being and quality of life for seniors.
  • Applicants should clearly outline their project goals, objectives, and anticipated outcomes.
  • Collaborative efforts involving multiple organizations are highly encouraged.
  • Grants are typically awarded for one-time projects, events, or ongoing programs.

Application Process

To apply for the FCHP Mini Grant Program, please follow these simple steps:

  1. Download and complete the grant application form available on our website.
  2. Include a detailed project proposal highlighting the need, objectives, budget, and expected impact.
  3. Submit the completed application and proposal via email to our designated grant committee.
  4. Applications are reviewed on a rolling basis, and decisions are typically made within 4-6 weeks of submission.
  5. If your application is approved, a representative from Kimwell Nursing Home will contact you to discuss further details and funding arrangements.
  6. Successful applicants will be required to submit periodic progress reports and a final report summarizing project outcomes.
